J-VON MEDICAL CENTRE Clinic Policy
I ____________ have read, and agreed to the following:
Online Booking:
J-Von Medical Centre offers online booking for phone calls, virtual appointments, and in-person visits through our website. We exclusively assist senior patients in booking appointments online.
Appointment Guidelines:
Due to the high volume of appointments, our office recommends that patients limit discussions to 1-2 medical issues per visit. In case of emergencies, our doctors will allocate additional time for discussion. However, for non-emergency matters, doctors will prioritize and address them within the allotted time. If necessary, a follow-up visit may be suggested to ensure thorough attention to each concern.
In our commitment to maintaining a timely schedule, we kindly request your understanding. If any delays occur, we appreciate your patience and request that you refrain from pressuring our front staff for timing adjustments. We acknowledge and value your time.
You should See Your Own Doctors:
Starting from 2024, our clinic will be transitioning to a system where patients will only be able to see their own family doctor. This change is reflective of the province's new medical model, which focuses on strengthening the patient-doctor relationship and improving the quality of care. As part of this commitment, please consult with your assigned family doctor for your care. Changing your family doctor within this clinic is not permitted.
Cancellation, No-Show, and Late Arrival Policy:
We understand that unexpected situations may arise, but to ensure that we can provide timely care to all our patients, we kindly ask for your cooperation with the following policies:
- No-Show Charge: If you miss an appointment without prior notice, a $75 fee will apply. This fee must be paid within two weeks and before scheduling your next appointment.
- Cancellation Policy: If you need to cancel or reschedule your appointment, please provide at least 24 hours’ notice. Cancellations made with less than 24 hours’ notice will incur a $75 fee, which must be paid within two weeks.
- Late Arrival Policy: If you arrive 15 minutes or more past your scheduled time, we will need to reschedule your appointment. A $75 fee will apply, and payment must be received within two weeks.
- Repeated Cancellations/No-Shows/Late Arrival: Patients who have three or more instances of cancellations, no-shows, or late arrivals may be discharged from our clinic.
Services Not Covered By MSP:
Some services offered at our clinic are not covered by the MSP and will require direct payment. For detailed information on fees for these services, please refer to our fee list. fee list.
Other Important Policies:
- Translation Services: Our clinic does not offer translation services and cannot accept medical reports that are not in English. Patients are responsible for bringing their own translator if needed and for ensuring any non-English documents are translated into English before submission.
- Urgent Appointment Policy: We do not offer urgent or walk-in appointments. If you require immediate medical attention, please visit an Urgent Primary Care Centre or Hospital Emergency Room. For non-urgent concerns, kindly book a regular appointment with us.
- Reports and Prescriptions: Lab results and prescriptions can only be discussed during an appointment with your doctor. Our clinic cannot release lab reports without prior consultation. Additionally, we do not prescribe narcotics or benzodiazepines.
- Contact Information: To ensure smooth communication regarding your care, please have voicemail enabled on your phone. If voicemail is unavailable, provide an alternative phone number and email. Kindly update us if your contact details change.
- Child Supervision: For their safety, children must be supervised at all times while in the waiting area and patient rooms.
- Cell Phone Use: To maintain a quiet and professional environment, we kindly ask that cell phone use be minimized in the reception area and patient rooms.
- Emergency Hospital Visits: If you visit a hospital for an emergency, please provide the attending doctor’s name rather than the clinic’s name. This ensures that your records are correctly linked within our system.
- Doctor Transfers: If your doctor relocates, we encourage you to continue care with them at their new office. In the case of retirement, you will need to find a new healthcare provider, as care under that doctor at our clinic will no longer be available. We will do our best to assist you with recommendations during this transition.
- Respectful Conduct: We strive to maintain a safe and welcoming environment for all patients and staff. Any form of rude, aggressive, or inappropriate behavior (including verbal abuse, threats, harassment, or property damage) will not be tolerated. Individuals engaging in such behavior may be asked to leave, issued a warning, or, in severe cases, discharged from our clinic.
We appreciate your understanding and cooperation in helping us provide the best care possible. If you have any questions, please feel free to reach out to our team.
